Role Description:
People with Physical Disabilities and Older People with Complex Needs deserve to live as independently as possible. Here in Surrey we are working to develop a creative, person-centred and innovative quality service to help people achieve this.

We are looking for a motivated and energetic professional to join our Surrey Heath Social Care Team to support people with a range of needs and enable them to access appropriate services.

As a qualified and registered professional (either in Social Work, Nursing or Occupational Therapy), you will be familiar with all aspects of social care including community care and related legislation. You will have a minimum of 2 years experience of person centred assessments and care planning, which enables you to identify people’s strengths in order to promote their independence, choice and diversity. As a senior member of the team you will have the ability to share these skills with junior members of the team through supervision and day-to-day management support.

Working collaboratively, you will make use of networks to obtain and share information and ensure that each person in our care receives the most appropriate support. As a strong team player you will have knowledge of the nature of health and social services and will be committed to developing inclusive services and achieving equality of opportunity.

Working flexibly you will work with a high degree of autonomy, prioritising work objectives including responding in a timely manner to changing or emerging needs. You will be actively involved in multi agency safeguarding processes. You will manage a complex caseload with support from your management team and colleagues.

As a senior member you will be part of the management team at Surrey Heath and will enjoy the prospect of undertaking management tasks and projects on behalf of the manager.
