Temporary one year contract
You will undertake a full scope and implement a pilot project of individual budgets for disabled children and young people in the transitions process, requiring short breaks and services across Health and Social Care.
This will include developing and implementing a transformation delivery plan aimed at mainstreaming this service available within Nottingham City and ensuring its provision meets the core offer, as identified in Aiming High for Disabled Children and the learning from other individual budgets pilots, both locally and nationally.
This role will involve you providing regular progress reports to the internal governance arrangements for Children’s Services, the NHS, Aiming High Steering Group, Strategic Partnership Children, Young People Executive Group and associated governance structures.
Experience of working with multi-disciplined teams and knowledge of the issues pertinent to disabled children and families in a multi-cultural environment, is essential.
A recognised professional qualification in Social Care, Health, Education or equivalent level (i.e. DiPSW) is required. Ref: ED4727A
